COVID-19 VACCINATION PROGRAMME FOR 12-15 YEAR OLDS

Dear Parent/Carer,

We have now received more information about the Covid-19 vaccination programme for young people aged 12-15.

You can also download and print a copy of this form from the link at the bottom of this page. The completed consent form should only be completed and returned for students that are aged over 12 years of age and under 16 years of age by the 16th of November.

For a Covid-19 vaccination to be given to your child, a printed, fully completed and signed copy of the consent form needs to be returned to school by Monday 11th October at the latest. A Covid-19 vaccine will only be administered if we have received a fully completed and signed paper consent form. If there is any disagreement between parents and their children regarding consent, this will be picked up by the vaccinations team. The local authority is working with relevant agencies to support the vaccination rollout. Any queries relating to the vaccination of the Students aged 12 to 15 will be dealt with by the vaccination team.

UPDATE (change of vaccination date): The completed consent form should only be completed and returned for students that are aged over 12 years of age and under 16 years of age by the 1st of November. A Covid-19 vaccine will only be administered to a student if we have received a fully completed and signed consent form. Covid-19 vaccinations will now take place on Tuesday 2 November (revised date).
